  “Comment author must have a previously approved comment.”

  Shouldn’t this setting block spam? I’m not approving spam.

  That setting requires all first-time commenters to be held for moderation, even spammers.

  If a comment was instantly approved, that would mean that at some point that comment’s email address had been approved.
