Transferring email subscribers to self-hosted site

I’ve done so much research and am super confused. Is there a way to transfer my subscribers and blog followers from my previous blog to my now self-hosted site? These are the sites:

THANK YOU FOR YOUR HELP!! However, I do not see a merge of the subscribers. I have only 39 followers, whereas I had approximately 500 on my former blog. Do I need to uninstall and reinstall something? :) -

We transferred all of your email and WordPress.com subscribers. The additional subscribers you had on your WordPress.com blog were social media followers who got your updates through the Publicize feature. You can set up Publicize on your new blog by following this guide: http://jetpack.me/support/publicize/
