Star Ratings that connect to Google and Facebook

    First, I want to create a form like this:

    Second, it takes me to a page containing this:

    Notice there are two links – one to Google and one to Facebook

    Finally (Google sample here) it lands the reviewer to my client’s Google Maps review page (pale background image) and displaying Google’s review form

    I would really value discussion about how I can achieve this, and also the same for Facebook.

    The normal WP review plugins I’ve found seem to be all about reviewing content on pages and blogs, which is not really what I’m after.
