Can someone help me with my email signup form on my website

My email signup form at the bottom of my website doesn’t seem to be working. I’ve used it before but now it doesn’t seem to even let you type anything or even use it. It’s suppose to be connected to my mailchimp account nd this is for my digital marketing class and it all just seems to be messed up and i don’t know where to start to fix it

I’ve taken a look at https://willyt843.com/ and it looks like the signup seen below is a button to the Thank you page, but not a form.
To use Mailchimp, I’d recommend using a Mailchimp block, as detailed here: https://en.support.wordpress.com/wordpress-editor/blocks/mailchimp-block/
I can see that Mailchimp has been connected here: https://wordpress.com/marketing/connections/willyt843.com
