PDF Form Submission Expert Opinion

    I am looking for some guidance on how to implement a form on my website. We are a healthcare provider and want to make patient forms available to be filled out digitally on our website, and upon submission they need to be emailed to the front desk in PDF format. I am using Elementor page builder for this project, which has a form builder widget that works excellently, but not PDF conversion upon send abilities.

    I have found Gravity Forms as a viable option, but the only issue is to customize forms (I need a very lengthly highly customized form) you need to use HTML/CSS/PHP, and I am not comfortable with PHP. They do offer form customization services but for the size of the form I need implemented it is upwards of $500.

    I checked out Caldera Forms, but they seem kind of shady. I found on there website info documentation about their PDF Form add-on, but it gets confusing from there. They seem to have changed the name of their pro plugin to WpSend (name change is never a good sign), and don’t have very much documentation about creating and implementing custom PDF forms. The plugin reviews within the past 2 years have alot of 1 star because of non existent support.

    I am wondering if anybody has implemented this sort of digital form that is sent to an email as a PDF, and what the recommended process is (plugins etc).
