Do I have to connect/purchase a domain before designing the site?

A free WordPress.com site will give you a domain using a WordPress.com subdomain, so you will get something like: yourwebsitename.wordpress.com, the “yourwebsitename” being a name of your choice (if it’s available). You can set that up and get started right away, no payment needed.
If you wish to use a domain name without the “wordpress.com” part then you will need to upgrade to a paid WordPress.com plan. You can read more about domains here: https://en.support.wordpress.com/domains/.

There is no need to connect or register a domain first, no.
You can experiment and build your site at that free address we give you when you create the site, even upgrade the site while still using the free address, and only switch the domain over to us once you’re ready.
Just keep in mind if search engines index your site at the free address (if it’s set to public) and you then add a custom domain, this could cause issues with SEO, as search engines will need to re-index the site at the new domain. But those should be temporary.
