  Add receiver/s Email/s of Contact Us form from Contact Us page via Domain email on that page. eg: (email visible only to moderators and staff) forwarded to designated person/s email address/es

  To clarify, you are wanting the option for a visitor to input their email address as they are filling out the contact form on your site?
