Comments stuck in moderation queue even though "Akismet cleared this comment"
Yep, just a few users. For most people, there is the expected behavior (first comment ever on site goes to moderation, remaining comments go through without any issue).

Are any of the other troubled users (the ones whose comments are moderated) actual users on this site?
Nope, the others should just be commenters. We do not require every commenter to sign up formally with an account
@sa40test that’s fine. Just wondering if that’s related. People who are site members never get stuff stuck in moderation (only in spam, in MH’s case.)
So all of the people who get stuck in moderation are not site members. Do all non-site-members get stuck in moderation every time?

Yes it seems that site members do not get stuck in moderation (except for one comment from MH that had 3 links, which probably put it there.)
Not all non-site members get stuck in moderation every time. Outside of J, FP, and MP, what happens is that the first comment will be in moderation, but all following comments will go through without being stuck in moderation. (That is the expected behavior, that all comments after the first is approved will automatically go through.)
OK. So J, FP, and MP are the only users having issues? I haven’t spotted patterns on their account but will keep looking.
I’ve only seen the pattern of ALL comments going to moderation queue for those three. It could be affecting more folks, but I haven’t noticed/seen that data in my records. For the other moderation queue emails I get, it’s just for first time commenters (or if someone has changed their name, even if their email is the same. I guess that also counts as first time commenter in the system?)
Can you find out whether FP and J are logged in when they are commenting?
I did notice both of them started getting moderated on March 12 and March 13.
J says he does log in. I’m asking FP if he does as well, so I’ll update when he emails back.
Just heard from FP that he also is logged in to comment. However, he gave additional information that MIGHT be meaningful:
“I do tend to be logged in to wordpress, but I always hit the “Change” button, as I want to give my real name (FP) rather than my wordpress name (A).”
Is it possible that because FP has a wordpress.com hosted site, that wordpress expects him to have a consistent name across blogs, and moderates him because he changes between sites?
J has a wordpress.com hosted site as well, but he uses the same name between his site and ours, so that’s not consistent
At this point we’re looking at a sneaky bug, so it could be anything. Does J change anything when he posts?
I now see two more posts in moderation from J…one with his typical name, and another with a variant that I’m guessing is his actual username on wordpress, rather than the nickname/display name.
Both got stuck in moderation queue.
OK. So both J and FP have at least once modified the fields when commenting, even while they are logged in.
I’d imagine leaving the fields alone doesn’t help at this point?
Whoops. Missed this thread for a bit.
Firstly, when J or FP leave the fields alone, they still go to the moderation queue.
Secondly, I wanted to announce that I am going to be turning back on some of our moderation queue filtering terms (that I don’t believe should affect these folks.)
That’s fine. And no worries on the delay, honesty I’m not sure what else to do with this and am mainly grasping for new ideas at this point.
Do we know whether FP and J’s comments are always going to moderation on other sites? Wondering if it’s their account or something triggered on your site.
J reported no issues with commenting on other WP.com blogs.
FP reported no issues with commenting on a blog that I know is a WP.com hosted blog. However, he did note that he has the same issue on another blog, but that blog is self-hosted WP.org, so I don’t know if there could be crossover there.
@sa40test I’ve just tried commenting with a modified name to see if I can duplicate this. You’ll need to approve my comment before I can test it properly. I’ll try to stay on topic :)
