<content:encoded><![CDATA[<p>There just seems to be something about July.  Everything at the hospital is new and enthusiasm is bursting.  It has made for a busy week in trying to keep up with the very best the internet has to offer.  I have expanded my usual offerings and still have to great posts I've left out.  If any of you are real die hard fans, subscribe to the nuggets from all over feed on the sidebar.  For those who just need a weekly fix of the world wide web's offerings, I present the best I've seen-</p>
<p>Regarding the Mind-</p>
<p style="padding-left:30px;">In a true display of a picture being worth a thousand words, <em>Medgadget</em> <a href="http://www.medgadget.com/archives/2008/07/tactile_illusion_works_just_like_visual_cousin.html">explains a sensory illusion by means of an optical illusion</a>, explaining how the mind causes it by the manner it integrates sensory input in general.  Very cool!</p>
<p style="padding-left:30px;">The New York Times has an vivid, eye opening report on<a href="http://www.nytimes.com/2008/07/15/opinion/15tue4.html?_r=1&#38;ref=opinion&#38;oref=slogin"> PTSD and the horrors of war</a>, as they tell the heartbreaking tale of private Dwyer.</p>
<p style="padding-left:30px;">At <em>Neuroscientifically Challenged</em>, Marc Dingman ponders the implications of a study showing that <a href="http://neuroscientificallychallenged.blogspot.com/2008/07/if-i-beat-up-robot-will-i-feel-remorse.html">the more human the appearance of a robot, the more likely we are to ascribe human intentions</a> to it.</p>
<p>Regarding the Soul-</p>
<p style="padding-left:30px;">In a post for the looking aiming square at the  the silver lining record books, Dr. Bob and <em>the Doctor is in</em> <a href="http://docisinblog.com/index.php/2008/07/17/welcoming-the-hypocrites/">sings the virtues of hypocrisy</a>, in a manner without hypocrisy or guile.  Insightful.</p>
<p style="padding-left:30px;">In a second post that moves somewhat along the same vein, Bruce Webster of <em>Adventures in Mormonism</em> shares a <a href="http://adventures-in-mormonism.com/2008/07/18/keen-religious-insight-from-a-comic-strip/">wonderfully insightful and artful comic strip</a> anwering common criticisms of specific faith and commitment.</p>
<p style="padding-left:30px;">Now that I have defended hypocrisy and commitment to a specific faith, here is an excellent post to balance things out.  At  <em>On Faith </em>, by Rabbi  Brad Hirschfield explaining the <a href="http://newsweek.washingtonpost.com/onfaith/brad_hirschfield/2008/07/the_power_of_faith_and_the_pro_1.html">problem with the perjorative terms we use to describe beliefs we find strange</a> while simply changing our tone for our own spiritual outlook.</p>
<p style="padding-left:30px;"><em>Shahrazad</em> has <a href="http://shahrzaad.wordpress.com/2008/07/17/a-red-rose-is-my-mecca/">stunning and beautiful poem</a> she shares with us about the wonder of creation and the prayer of a Muslim, stating  "A Red Rose is my Mecca.".</p>
<p>Regarding the Body-</p>
<p style="padding-left:30px;"><em>NPR</em> reports a new discovery that H Pylori, the stomach bacteria responsible for ulcers, has some good in the body as well.  It turns out <a href="http://www.npr.org/templates/story/story.php?storyId=92677188&#38;ft=1&#38;f=1007">people with the bug have less asthma</a>.  My, My medicine is getting more complex all the time.  I can't keep the good bugs and the bad ones straight anymore.</p>
<p style="padding-left:30px;">In the amazing medical gadgetry department, <em>Medgadget</em> presents the <a href="http://www.medgadget.com/archives/2008/07/pillcam_used_to_diagnose_crohn_disease.html">Pill Cam, a pill sized camera</a> that has been used to identify and diagnose Crohn's disease, an inflammatory bowel disorder.  Who ever new a pills eye view of the gullet could be so useful.</p>
<p style="padding-left:30px;">At Radiology Picture of the Day, a fascinating (to me, anyway) picture of an <a href="http://www.radpod.org/2008/07/14/appendicitis-in-pregnancy/">MRI showing appendicitis in pregnancy</a>, showing just how the appendix migrates to where doctor's don't expect it to make room for a bun in the oven.</p>

<p style="padding-left:30px;">At <em>Revolution Health</em>, Dr. Val and the voice of reason has an amazing story about the a <a href="http://www.revolutionhealth.com/blogs/valjonesmd/not-lost-to-follow-u-14790">90 year old retired pediatrician who remains very devoted to his patients</a>.  As he described how he openly prays for his patients, I was struck by how he is truly the product of a bygone era.</p>
<p style="padding-left:30px;">At<em> Musings of a Distractible Mind</em>, Dr. Rob ponders the <a href="http://distractible.org/2008/07/16/self-assured-destruction/">human appetite for self destruction</a>, embodied in its extreme by Lesch-Nyhan syndrome in a very thought provoking post.</p>
<p style="padding-left:30px;">Dr.Deb has here finger on the pulse of the latest technology for Autistic spectrum disorders, built on the idea that <a href="http://drdeborahserani.blogspot.com/2008/07/portable-hugs.html">what they really need is a hug</a>, as she describes a pressure vest designed to increase touch senory input, or self stimulation and calm patients with difficulty integrating their sensations.</p>
<p style="padding-left:30px;">In the spirit of abhorrent breaches of ethics and privacy that I oppose body, mind, and soul, here is a report at <em>GNIF BrainBlooger</em> describing the politics behind <a href="http://brainblogger.com/2008/07/17/the-ethics-of-selling-prescription-data/">selling physicians prescribing habits to pharmaceutical companies</a>.  Write your congressman today, this needs to end.</p>

<p style="padding-left:30px;">Dr. Rob at <em>Musings of a Distractible Mind</em> was on a roll this week.  Here is a hilarious post looking at the absurdity of the insurance industry and <a href="http://distractible.org/2008/07/16/coding-by-accident/">the lengths they go to find the perfect code</a>.  If only their dedication to their actual customers and general health care quality was this complete.</p>
<p style="padding-left:30px;">The <em>Clinical Cases and Images Blog</em> <a href="http://casesblog.blogspot.com/2008/07/goodbye-to-worlds-oldest-blogger-age.html">bids a fond farewell to the world's oldest blogger</a>, Olive Riley of Sydney, Australia, who passed away at 108.  She truly exemplified the motto, you are never too old to pick up a new hobby,</p>
<p style="padding-left:30px;">Here is an amazing performance by a group of high school kids calling themselves <em>the Wrong Trousers</em>, playing what I guess is now a classic oldie, from my childhood (Am I really that old?), "<em>Video killed the Radio Star</em>" and playing it 90s style old school, unplugged.  I more than <strong>LIKED</strong> it, this is simply brilliant!</p>

<content:encoded><![CDATA[<div class="post-body entry-content">The by-now common sense view that exercise delays the onset of many diseases including Alzheimer's has been supported by a study in the latest edition of Neurology (1). 64 people without dementia were compared with 57 people with early onset Alzheimer's disease. The exercise capacity of subjects was assessed (using a measure of their use of oxygen) as were the brain volumes of the subjects. Essentially the results showed that if a person had Alzheimer's and didn't do very much exercise, their brain volumes were smaller than someone with Alzheimer's who did a lot of exercise. In the people without dementia this didn't seem to hold true i.e. exercise didn't seem to be related to brain volume. The conclusion was that if your going to get Alzheimer's, you'll probably have a better prognosis if you do more exercise. Why? - because having a larger brain volume gives you a better prognosis. So the evidence piles up in favour of us doing exercise!</p>

<title><![CDATA[ADD Assessment  - part two]]></title>

<content:encoded><![CDATA[<p>So - the other day I went for my second of 4 appointments for my Adult ADD Assessment.  It was very interesting to say the least.  They had me doing numerous neuropsych screens to determine if at some time in my life there were any neurological problems ... i.e. mini strokes, etc.  I've never had any concern that this was the issue, but it was part of the whole assessment and I guess it's good to have it ruled out.</p>
<p>During the neuropsych screen I had to do things like repeat back a tapping rhythm and sticking out my tongue in three different positions as quickly as possible for 10 seconds - yeah, that was strange and the test administer prefaced it as "this is going to seem strange, but bear with me"... but I think I passed that part with flying colors.</p>
<p>After the neuropsych screen, I then moved on to memory testing - having to remember a drawing and re-drawing what I remembered at 3 different points during the appointment.  I don't even remember half of what I had to do that day - how ironic, huh?  The last few years I have noticed a big decrease in my memory.  It really scares me a bit.  I can ask my husband or kids a question and then two minutes later I ask the same question again.  I think the problem lies within my short term memory.</p>
<p>There were a ton of math equations and word problems.  I feel as though I bombed those... fractions, square root stuff and more... thankfully I know I answered what the symbol for Pi was ... 3.14.  I attempted quite a few fraction problems, but I am not confident that I completed them correctly.</p>
<p>At one point I sat with headphones on and the CD test they had me listen to had me repeat back sentences by the narrator, but at times during that test, there was background noise - like people talking...my downfall.   Then the narrator said 2 different sentences at the same time and I had to repeat back the one that I heard from the right ear and vice versa with the left.  My brain was completely fried after that test.  I had to concentrate really, really hard to hear the sentences above and beyond the background noise.  It was like being at a busy restaurant with my husband and trying to focus on our conversation - I get SO distracted, that he used to think I was eavesdropping on other peoples conversations, when in fact, I just couldn't focus with all the background noise and was squirming in my seat!</p>
<p>I go back again on Tuesday for a QEEG Screen that will test/measure brain waves.  This is a bit controversial at the time because there is research that shows ADD brains look different than non-ADD brains.  I think there are a lot of non-believers in this form of testing for ADD.  </p>
<p>I figure that all bases are being covered with all of this testing that I am going through - and the best part of it is that my insurance is paying for most of it.  I had wanted to go to this particular clinic a few years ago, but I had different insurance at the time.  So, I feel as though if this assessment doesn't show a final result of ADD or possible learning disability then maybe I can just move on with my life... or maybe I will once again go back to thinking that I really am stupid.  I can't bear to think like that.</p>

<p>Irvine (pte/03.07.2008/10:00) - Sehr alte Frauen sind offenbar besonders anfällig für Demenz-Erkrankungen. Laut einer Studie der University of California <a href="http://www.uci.edu/" target="_blank">http://www.uci.edu</a> sind fast die Hälfte aller Frauen über 90 Jahren davon betroffen. Die Analyse der Daten von 900 Menschen über 90 Jahren ergab, dass Männer gleichen Alters weit seltener an Demenz erkranken.</p>
<p>Frauen mit einer höheren Bildung sind dagegen scheinbar einem deutlich geringeren Risiko ausgesetzt. Die Ursachen sind derzeit noch nicht erforscht. Ältere Frauen verfügen jedoch über ein höheres Risiko eines Herzanfalls oder eines Schlaganfalls. Beide gelten als Risikofaktoren für Demenz. Details der Studie wurden im Fachmagazin Neurology veröffentlicht.</p>

<p>Das Team um Maria Corrada wies nach, dass die Wahrscheinlichkeit einer Demenz-Erkrankung sich bei Frauen nach dem 90. Lebensjahr alle fünf Jahre verdoppelte. Bei Männern konnte kein vergleichbares Phänomen festgestellt werden. 45 Prozent der Teilnehmerinnen an der Studie waren betroffen, aber nur 28 Prozent der Männer. Eine Studie des King's Fund <a href="http://www.kingsfund.org.uk/" target="_blank">http://www.kingsfund.org.uk</a> legte kürzlich nahe, dass die Belastung durch Demenzerkrankungen in den nächsten zwei Jahrzehnten durch die Alterung der Bevölkerung drastisch ansteigen wird. Allein in Großbritannien sollen sich dann die Kosten für die Gesundheitsversorungen auf mehr als 35 Mrd. Pfund erhöhen. Die Anzahl der Demenz-Patienten soll die 900.000-Grenze überschreiten.</p>
<p>Es bestehen Befürchtungen, dass Demenz-Erkrankungen das Gesundheitssystem vor eine völlig neue Herausforderung stellen könnten. Bisher gibt es nur wenige Studien, die sich spezifisch auf Demenz-Erkrankungen bei sehr alten Menschen konzentrieren, berichtet BBC.</p>
<p>Die zunehmende Lebenserwartung bedeute jedoch, dass es sich um eine rasch wachsende Bevölkerungsgruppe handelt. Weitere Studien sind zu dem Ergebnis gekommen, dass Demenz-Erkrankungen bei Männern und Frauen zwischen 65 und 85 Jahren immer häufiger auftreten. (Ende)</p>

<p style="text-align:center;">One of the features of humankind that has long been thought to be unique to us is the theory of mind.  This is our ability to deduce what another might be thinking, a critical base for such behaviors as for empathy, socialization, even battle and strategy.  It is so prevalent and so innate that we often do it without realizing it, anthropomorphizing machines, televisions, the computer, animals, even the clouds and the waves.  Imagine a world where no one did this.  Imagine for a minute not even realizing that the people you interact with daily have their own minds inner workings and dealings.  This is the world of Asperger Syndrome. <a href="http://www.communicationcurrents.com/NCANewsletter/files/ccLibraryFiles/Filename/000000000028/neuron.jpg"><img class="alignnone" src="http://www.communicationcurrents.com/NCANewsletter/files/ccLibraryFiles/Filename/000000000028/neuron.jpg" alt="" width="500" height="375" /></a></p>

<p>   Asperger syndrome is the very high functioning end of the Austistic spectrum.  By definition their intelligence is normal, usually even well above normal, but their capacity for reading nonverbal cues, socializing, and empathizing is severely impaired.  We don't understand the basis for this entirely and certain different theories abound, but it appears Asperger individuals lack the firing of what is known as the mirror neuron.  This is when a part of the brain usually responsible for movement or planning  movement activates just by watching the same movements.   We recognized what another is doing and actually activate our brains as though we were doing the same thing ourselves.</p>
<p>    Patients with Asperger's are terrible at reading faces, recognizing nonverbal cues, or recognizing figurative language.  Patient's will say that when in a conversation, they see everyone else as Robots.  In addition, they tend to have very single minded ability to focus and will delve deeply into figuring out a phenomenon that interests them.  They can talk at length to anyone about their interests, even while boring the audience completely to tears.  They aren't great at human interaction and therefore much prefer reading on a subject to being taught directly.</p>
<p>   They seem to have difficulty ignoring sensation of any type.  They can be very disturbed by ampient noise or by trying to talk with more than one person at the same time, shirt tags will drive them crazy, outside noises are a problem and they can be very inflexible and develop stereotyped behaviors they use when overwhelmed by their environment.  They are extremely sensitive to texture and are picky eaters.  The stress of overstimulation causes them to withdraw into what they can carefully control and may lie behind their repetitive, self calming rituals.  Interacting with us unpredictable standard human beings can actually be stressful because of our seeming randomness.</p>
<p>     This makes Asperger's syndrome and Social anxiety disorder look very much alike, even though internally and mechanically they are worlds apart.  As a social phobic, I care too much about what everyone else thinks, to the point it can be paralyzing.  Nevertheless, I crave human connection and company.  It's a curse.  There are many days Asperger's would seem to me to be so much better.</p>
<p>  They are typically first class researchers and likely overrepresented in the scientific field.  Emotions do not typically interrupt their inquiry.  They often describe themselves as strictly logical, and appealing to logic is critical in teaching them social skills.  They have to understand why it is important to interact and have to be given detailed instructions regarding nonverbal cues, manners etc. that they learn by repetition in order to function with other people.</p>
<p>    In a very real sense patient's with autism are irremovably at the center of their own universe.  They lack capacity to be anything other than self centered.  Yet they can undoubtedly can make very important contributions to society.  They are true explorers of knowledge and when something strikes their interest they have an unbroken focus in learning all about it.  This is uniquely human.  It separates us from the animals, which also have sociality, nonverbal communication, and hierarchical societal interaction, after all. </p>
<p>     At the same time they challenge my entire conception of what it is that makes us human.  A complete lack of empathy, if found in all of us would quickly cause society to disintegrate.  It is central my job as a competent (patient seeing) physician.  I do wonder about many of my neurosurgeon and pathologist colleagues however.  ;)   So many of my central spiritual ideals, compassion, mercy, understanding, are foreign and possibly beyond the grasp of these individuals.</p>
<p>    Saying this, I can't believe that Asperger patients are psychopaths.  They are quite peaceful, pragmatic, and conscientious.  They are the picked on, not the bullies.  I am not entirely sure why this is, but my experience tells me it is true.  They tend to feel very strongly that they are fine the way they are, and disability advocate that I am, I agree.  Like any of us, they have strengths and weakness, with focus, curiousity, and concentration being incredible and empathy and human understanding being much weaker.  I think the bottom line is that they can learn, and learn well, and that covers all manner of weaknesses by leading to growth and development.</p>

<p class="MsoNormal" style="text-align:justify;margin:0;"><strong><span style="font-size:11pt;font-family:Arial;">Tolosa Hunt Syndrome(THS) is a painful opthalmoplegia caused by idiopathic granulomatous<span>  </span>inflammation of cavernous sinus or superior orbital fissure. It is a diagnosis<span>  </span>of exclusion with an estimated annual incidence of one case per million per year(1U).<span>  </span>We present a patient who presented with isolated 6th nerve palsy and was diagnosed for THS. </span></strong></p>
<p class="MsoNormal" style="text-align:justify;margin:0;"><strong><span style="font-size:11pt;font-family:Arial;"> </span></strong></p>
<p class="MsoNormal" style="text-align:justify;margin:0;"><strong><span style="font-size:11pt;font-family:Arial;">Case Report</span></strong></p>
<p class="MsoNormal" style="text-align:justify;margin:0;"><strong><span style="font-size:11pt;font-family:Arial;">A 50 year old Hispanic female with no relevant medical history presented with isolated pain in the left eye since 2 weeks and diplopia of sudden onset.<span>  </span>She denied fever, trauma, recent surgery or travel. On examination, patient had lateral gaze paralysis , decreased visual acuity due to poor gaze fixation and diplopia.<span>  </span>She did not have any other<span>  </span>cranial nerve deficits. The rest of the systemic examination including the<span>  </span>nervous system was unremarkable. MRI brain did not show any focal lesions. Lumbar puncture, Antibody assays for Connective Tissue Disorders, Lyme’s Disease ,Serum Protein Electrophoresis and other laboratory examinations failed to reveal any abnormality. After consultation with neurology and ophthalmology and their thorough investigation, patient was diagnosed for THS. She was started on<span>  </span>prednisone 1mg/kg dose daily with resolution of symptoms in a few days. </span></strong></p>
<p class="MsoNormal" style="text-align:justify;margin:0;"><strong><span style="font-size:11pt;font-family:Arial;"> </span></strong></p>
<p class="MsoNormal" style="text-align:justify;margin:0;"><strong><span style="font-size:11pt;font-family:Arial;">Discussion</span></strong></p>
<p class="MsoNormal" style="text-align:justify;margin:0;"><strong><span style="font-size:11pt;font-family:Arial;">THS is described as “episodic orbital pain associated with paralysis of one or more of the third, fourth, and/or sixth cranial nerves”(6U). It is seen at almost any age without any gender or race discrimination. MRI shows changes in 44% of case only. In most cases it is diagnosed when all other vascular, neoplastic and inflammatory causes of painful opthalmoplegia are ruled out. Orbital pseudotumor is an important differential diagnosis as it differs only in relation to anatomic localization.<span>  </span>Remission is possible in 40% of cases<span>  </span>with ipsilateral, bilateral or contralateral involvement. Steroids are the mainstay for treatment. Initially given high dose upto 80mg daily, gradually the doses are tapered off.<span>  </span>If need be other immunosuppresants like methotrexate,<span>  </span>cyclosporine, mycophenolate mofentil and azathioprine can be tried.<span>  </span>Usually the radiological improvement lags behind the clinical response to therapy. Though essentially a benign disease,<span>  </span>neurological deficits are possible requiring long term<span>  </span>immunosuppresion. The purpose of this<span>  </span>case report is to reinforce the fact that THS can be a severely debilitating disease with dangerous consequences but at the same time if diagnosed appropriately, it can completely cure the patient. Thus THS should be at the back of the mind of all internists when a patient presents with similar signs and symptoms.</span></strong><strong></strong></p>

<p> <strong><span style="color:#ff6600;">SURGERY for psychiatric disease is experiencing a rebirth </span></strong>as neurosurgeons employ deep brain stimulation techniques and show an improved understanding of limbic circuitry obtained from functional cerebral imaging. These treatments have now been welcomed by psychiatrists and their patients, who have been without surgical options for decades. The rampant use and abuse of frontal leukotomy in the mid 20th century remains a sobering memory.<br />
 WE ARE PLEASED to introduce the July 2008 issue of Neurosurgical Focus, which is devoted to the topic of psychosurgery. Tremendous advances in our knowledge of limbic circuitry and neurochemistry have invigorated numerous investigative centers around the world to employ rigorous and honest scientific methods in the study and treatment of patients with mood and anxiety disorders.</p>
<p> The first series of papers in this issue is devoted to the history of psychiatric surgery. A paper by Manjila and colleaguesnotes that an unknown pioneer in the field, Dr. Gottlieb Burckhardt, performed the first human experiments for psychological disorders by excising regions of the cerebral cortex in the 19th century—for which he was roundly criticized. Soon thereafter, the psychiatric environment<br />
 changed and became more favorable so that Dr. Egas Moniz was able to perform the first human prefrontal<br />
 leucotomy, as noted in the paper by Wind and associates. Elias et al. observe that much of our current understanding regarding the parallel limbic circuitry of the striatum was obtained later from the neuroanatomist Dr. Lennart Heimer, who developed a track-tracing method and used it to discover<br />
 the pathways of the ventral striatum.</p>
<p> We are also very pleased to present several contemporary clinical articles pertaining to <strong><span style="color:#ff6600;">neurosurgical procedures for mood disorders including depression, anxiety, addiction,<br />
 and aggression.</span></strong> These articles form an excellent reference set for future clinical trials and studies. The clinical section begins with a comprehensive discussion from two renowned experts (Abosch and Cosgrove) regarding the biological basis for neurosurgical intervention. This written work alone serves as an authoritative reference for what is currently understood about the biology of depression, and<br />
 we are pleased to provide a podcast as well (http:// nsforum.org/) with further thoughts from the authors.<br />
<strong><span style="color:#ff6600;"> Contemporary ablative procedures are still quite effective, </span><span style="color:#ff6600;">and Dr. Eljamel discusses his experience with stereotactic cingulotomy and anterior capsulotomy. Addiction, a disorder<br />
 so constantly present in our society, is reviewed by Visser-Vandewalle and colleagues as a potential target for future neurosurgical techniques. Last, Dr. Fountas reviews the management of aggression using stereotactic amygdalotomy</span></strong>.<br />
 The final section of the issue is devoted to laboratory investigations in the field of psychiatry. It is a true sign of the future to observe neurosurgical groups such as Wekenhuysen and colleagues actively studying <span style="color:#ff6600;"><strong>psychiatric conditions such as anorexia nervosa</strong></span>. This is only the beginning of new developments in this field, and we anticipate much more interest in the science and treatment of psychiatry byneuroscientists and clinicians. <title><![CDATA[The neurology of aging]]></title>

<content:encoded><![CDATA[<p>Is aging normal or abnormal/pathological? No one quite knows the answer to that question. What we do know is that as we age, neurological disorders become increasingly common. These may range from well defined neurodegenerative diseases like Alzheimers dementia, Parkinson disease and amyotophic lateral sclerosis to other less well defined conditions like gait disorders, "balance problems", "forgetfulness and senior moments" and increased propensity to falls. Strokes become more common in the aged brain vessels.</p>
<p>As life expectancy increases and more and more people live past the eight decade, neurological conditions become common and account for substantial morbidity and mortality in the oldest old (above 85). Earlier when the life expectancy was in the 60s, we did not see so much Alzheimers dementia, Parkinson's disease or brain tumors. People died of other "natural" and "unnatural"  causes before the brain showed clinical manifestations of neurodegeneration.</p>
<p>Is it the norm that as we age, a substantial majority of us are destined to develop dementia?  Clinical studies have clearly shown that Alzhemier disease pathology increases with age and the incidence of the disease becomes increasingly common as one goes past 85 (the oldest old). Other studies suggest that though not all the oldest old show clinical dementia, a substantial majority have cognitive difficulties if carefully tested for at the bedside.</p>
<p>Why do neurological conditions become more "common" as we age and can we do anything to alter this? Many theories have been propounded. Increased amyloid deposition in the brain has causal association with Alzheimers dementia, in the same vein deposition of iron in the basal ganglia has been postulated to cause various basal ganglia pathology. There is increased oxidative stress in the "aged" brain which leads to free radical formation and damage to the cellular DNA. Genes get switched off or on triggering the disease process. A lot still needs to be learned about the neurology of aging.</p>
<p>While the mechanisms are still been elucidated, is there anything which we can do to change our "risks". In the absence of good studies most of the data is open to interpretation. Aspirin prophylaxis, modification of microvascular and macrovascular risk factors like hypertension, diabetes mellitis and dyslipidemia (high "bad" cholesterol) all seem to be reasonable interventions. Obesity and sedentary life styles are bad for the brain too. Regular physical as well as brain exercises (neurobics) keeps the brain healthy and increases neuronal reserve. The role of anti-oxidants like coenzyme Q10 and alpha lipoic acid is still been defined. As they are relatively innocuous and free from side-effects, I would recommend them on a case by case basis. Episodes of major depression "hurt" the brain and aggressive treatment with anti-depressants should be initiated early rather than late.</p>
<p>The neurology of aging remains an uncharted territory but there is hope yet.</p>

<content:encoded><![CDATA[<p>Pat Metheny used to begin every concert just like this song - though probably not for any reason that he knew, but, then again, only he knows:  the sound system would play  THE  WAY OF THE  WORLD by Earth Wind &#38; Fire , a song at the speed of loneliness.  Then for over 15 years the lights would go down and the speed would literally BREAK - in Pat's  case to PHASE DANCE, which traveled through the speeds of renewal, enthusiasm,  and comfort.</p>
<p>Macca here  does the same thing.  A neat trick - try playing Eminem's STAN or Dido's THANK YOU and then *break* the speed.</p>

<p>I didn't think so.  (But you have to admit, the parts of the brain can sound mildly erotic... massage my amygdala, Magda. Oh-ho-ho-<em>yessh!</em>)</p>

<p>熱和涼是感覺,而冷和暖是溫度,一直以來都以為它們是指同一件事,因為天氣冷所以覺得寒,因為天氣暖所以覺得熱。直至後來有一次,翻開了高等物理的課本,才發覺當風在動時,代表空氣粒子動能增加,換句話說,效果和溫度上升一樣?因此出現了矛盾,溫度上升反而令人更涼快,何解?是不是說身體的感覺並不真實反映外界的環境?<br />
後來才知道,風之所以令人涼快,不是在於風的本身,而在於風帶動空氣粒子做成的效果,因為天氣熱,人的生理反應是會流汗,而空氣粒子動能增加的後果是增加汗的蒸發速度,汗的蒸發的效應就是被空氣的熱能啟動了液態轉氣態的物理反應,而在過程中反而吸入了比啟動能更多的熱能,因此做成身體溫度下降的效果。因此,是身體本身變得比空氣涼快,而神經系統產生感覺所反映的不是過程的本身,只是過程的結果;而感覺反映的不是外界的環境的變化,而是身體內在的環境的變化。因此科學書本上說人的感覺不敏銳,有誤差,只是對了一半,而是因為人的感覺系統只是為了人類的生存而設,不是為了準確偵測外在的環境,熱和涼是人的神經系統經過複雜的運作,而外界環境又和生物反應不停互動反饋,理性思維為了表達這一過程而用語言創造出來的慨念物件,理性思維的前提是自我意識,而自我意識又是神經系統活動的結果。我們本來是生活在一個抽象世界,而所感知的世界其實是我們神經系統整體活動所形成出來的自我意識所建構出來的心理世界,真實的物理世界我們永遠接觸不到
