Vamana - induced vomiting
Virechen - purgation (therapeutic laxative)
Basti - enema
Nasya - nasal detoxification
Rakta Moksha - blood detoxification

In this blog series, we will explore each method of therapy, and why it is used.

________________________________________________________________________

Basti

Basti is the term in Ayurveda used for therapeutic enemas. It is used to treat Vata disorders such as: constipation, sexual disorders and joint pain.

This highly therapeutic method uses mixtures of oils and herbal preparations in liquid form. When introduced into the body, these mixtures help alleviate and ease Vata imbalances. Medication given rectally penetrates deeply into the body's tissues, and is therefore very effective.

To the western mind, the mere thought of this treatment is extremely unpleasant. But when performed by a qualified practitioner in a comfortable setting, the procedure itself is not all that bad. In fact, this form of treatment will often work when everything else has failed.

This coming Monday's Blog: Nasya (Therapeutic treatment via the nasal passages)

Was macht also der schlaue Max?
Keine Chance also, an das Dictionary ranzukommen.

Naja, we will see...

Kanpur and Lucknow computer dealers who have for years been selling proprietary operating systems ar

Kanpur and Lucknow computer dealers who have for years been selling proprietary operating systems are suddenly realizing that they themselves know very little about Ubuntu Linux operating system for desktops. On top of that, they have not invested in teaching their own support staff how to install Ubuntu or Kubuntu on customer laptops and desktops - that is customers who want to work with the best legal operating system already available in the markets. Many of these computer dealers have not even found the time to read the installation instructions from the Ubuntu website.

Naturally these assembled computer dealers associations in Kanpur and Lucknow feel that first they have to learn Ubuntu themselves - and then - they have to teach their staff how to install Ubuntu or Kubuntu in one hour. So it seems that school children will now have to teach these outdated computer dealers and hardware box vendors the fine points of Ubuntu Linux installation on laptops and desktops. One wonders when the the computer assemblers will start giving themselves a crash course in Ubuntu and Linux so that they can install it on their customer computers in just one hour.

But go and see the computers of children in Kanpur and Lucknow from schools such as Methodist, Jaipuria DPS, Christ Church - these kids know much more about the ease of Ubuntu installations than these hardware vendors - many of whom do not even have free CD to give to the customer just in case the customer tells them just to give them a price for the hardware box. For after all if children can install Ubuntu from CD by themselves, so can customers. And then maybe finally the staff of the hardware vendors will also learn how to install Ubuntu.

Many children of course prefer Kubuntu Desktop version because the KDE desktop is better to look at for those who only know how to send emails and chat on the Internet.

And last, the computer dealers will themselves shyly one afternoon decide to learn it themselves.

Where to download Ubuntu from ? http://www.ubuntu.com/getubuntu/download

Well guys, finally tomorrow is the last day of my PSM rotation. Kyaa…PSM nahin jaante?? Well i

Well guys, finally tomorrow is the last day of my PSM rotation. Kyaa...PSM nahin jaante?? Well it’s not your fault actually. This field has so many names, just the way it has so many national programmes, policies and guidelines. This monster is also known as preventive and social medicine, community medicine, public health and few other weird names, mentioned in uninspiring detail in the bible of this subject: Park's. Throughout my MBBS I wondered why this book is called Park when in fact it’s written by opnaa baungalii baabu?? Anyone with suggestions? Anyways, back to the PSM experience.

So my rotation. Well since it is community medicine, we are supposed to work in community. So this meant separating from our stronghold of hospital environment and suddenly we find ourselves in unconquered territory where we are no longer the bullies :-D , where jee-hazoori of patient is foremost and prescribing meds on patient discretion the norm rather than exception!! So, the 2 months of my PSM rotation were divided into 4 "lovely and romantic" fortnights. First in slums of ITO (Balmiki Basti...the great sage Balmiki used to take medicines from our center), then in rural fields of Barwala (famous for its numerous Bars and pubs, you see Bar...Waala), followed by yet another urban slum of Gokulpuri (situated near Yamuna, the place where Krishna used to come for seeking cure for his ever so frequent burning micturition, considering his not so safe sexual practices :-D, and from where Taj Mahal can be seen in Delhi...that’s the Majnu Ka Tila gurudwara, which gives illusion of a mini Taj from distance) and lastly Delhi gate which is more famous for its delicacies than our humble health center, which was discovered by Indiana Jones (what??? you haven't heard about Indiana Jones and Lost PHC...the only best movie of all times!!! Dude, you need therapy).

Well, one thing I accept. No matter how redundant and boring the job might be, we got scot-free by 2pm almost always. But then we were so sapped by Delhi heat and frustration that at least I used to doze off almost every noon. This makes me feel guilty even today, but then I am just a 60kg, lean, thin fellow and I get tired easily!!! So it’s excusable, isn't it!! Now what I used to do in the posting. Hmmm...interesting as soon as I come to know about it, I will write back. He..He..Ok, on a serious note here is a not so serious guide to 4 PSM centers and their "workload"

Balmiki Basti:

Step 1: Reach center at 10 am. Snail pace recommended.

Step 2: Settle down. Please take your time.

Step 3: Listen to occasional bhaashan from the resident. Inattentiveness highly desirable.

Step 4: Wait for the one or two patient who comes to center. Aree yahaan bhi dawaa khanna hai...chalo dikhaa letaa hoon!!

Step 5: Scribble down something on the "prescription" only at discretion of resident. Your job is just to write and listen to his/her daant. Feel depressed about your apparent lack of knowledge.

Step 6: In free time, play Dum-Sharrat (the game where one guesses movie names by signs, yaar...)

Step 7: Pack up and give attendance. Maximal attention advised. Altruistic behavior strongly discouraged.

Step 8: Run off as if there is no tomorrow.

Step 9: Reach college by noon and go home.

Step 10: Repeat next day.

Barwala:

*** Barwala samosa's are awesome. Must be relished daily***

Step 1: Reach college to catch the Barwala RTV by 9am.

Step 2: Chill with friends on the backseat of RTV.

Step 3: Reach Barwala after 1 hour of bumpy ride. Heavy meals recommended, as studies have shown better digestion and food grinding in subjects on Barwala RTV than those not.

Step 4: Rush to loo. Urine becomes frothy. Etiology uncertain?? (or do I know!!)

Step 5: Settle down. Seat far away from resident and towards the other side of table and 3 rd last from corner strongly advised. Better fan airflow, minimal chik-chik and easy dozing...

Step 6: Be extremely cautious while writing prescription as pharmacist at center has been specially trained to be anti-Intern.

Step 7: Send out few of your friends after 1 hour of hectic work (really, its hectic) for procuring those samosa's...

Step 8: Relish samosa and ladoo and chips in the AC room, specially designed for this purpose. Full satisfaction of taste buds mandatory. Don't leave until satisfied.

Step 9: Pack off and attendance.

Step 10: Ride back to college. Open your tiffins (yaar samose to sirf appetizers the) and go on a bumpy eating frenzy.

Step 11: Food digestion guaranteed. Go home for lunch at 2. (ab baar baar appetizers khaake, bhoom to lage gi naa!!!)

Step 12: Repeat every day.

Step 13: Consult orthopedician for slipped disk. Frothy urine disappears mysteriously...

Gokulpuri

Step 1: Curse your luck. This center is much closer than Barwala and the ride is relatively smoother. This means more work and more fit doctors.

Step 2: Chill with friends on back seat of RTV. Yamuna view is spectacular and so is Taj.

Step 3: Reach Gokulpuri. The halaal meat shop next to center has a different daily attraction. Must see.

Step 4: Listen to center incharge's stupid jokes. Laughing with him highly recommended, lest he will pester you with horrendous questions. (His laugh by the way resembles starting a rickety Bajaj chetak scooter!!)

Step 5: Start the dukaan. This center is very heavy!!! Differences between friends bound to rise.

Step 6: Must take a Jaapani pankhaa with you. No fan for interns, sob..

Step 7: Finish off the patient load. No pesking pharmacist though!!!

Step 8: Return to the incharge's room for the noon dose of laughter (if taken casually) or boredom and sadness (if "le liyaa dil par").

Step 9: Return to college. The food fest should continue as in Barwala, though no refreshing samosa's as in Barwala.

Step 10: Repeat next day.

Delhi gate

Step 1: Depart from college at 10 am. Hiding steths in pockets strongly advised, as many of our previous Interns have disappeared mysteriously!!

Step 2: Control you urge to eat kachodis, gullab jamuns, chaat et al. on your way.

Step 3: Offer daily prayers to your god of choice. (Hanuman prayer strongly advised. Best returns!!)

Step 4: Climb the stairs to reach center. The only place where you feel on top of community.

Step 5: Highly recommended to sit at the extreme corner of room. Most patients are of URI and it will get boring to prescribe CPM and PCM again and again.

Step 6: The center's resident is relatively better than other PSM folks. Warrants detailed investigation for a possible mutation.

Step 7: The pharmacist is another bright student of anti-Intern training academy. His mole on cheek looks really irritaing. Control your urge to bash him, though he does go home alone....

Step 8: Pack up at 12:30pm, longest working hours in PSM.

Step 9: Must stop over daily at delicacy shops in and around Daryaganj for quality lunch. Kulfi falooda and Badam milk near Mithaas highly recommended.

Step 10: Return to college and home or make a plan to visit some place nearby for an evening with friends...

Step 11: Repeat next day. Don't come if you bashed the pharmacist.

So that’s it guys. Hope you liked my STEPS approach to PSM centers. There is still another month of PSM posting, but that will be after 3 months. Till then I will begin (and hopefully enjoy) my surgery, psychiatry and one undecided rotation. Overall PSM was fun and I did manage to do a lot of meaningful stuff in these 2 months (will write about those later), but then I have seen better days!!!

And certainly there are much better way to spend scorching Delhi summers than PSM!!

What is Anorectal Malformation ?

During early development of a baby in the womb, the urinary tract, the reproductive tract and the digestive tract all exit the pelvis through a common opening. Normally, separation of these structures occurs. In females there is complete separation of the three systems so that the urinary tract opening is farthest forward (the urethra), the reproductive tract exits in the middle (the vagina) and the digestive tract exits farthest back (the anus). In males the urinary tract and the reproductive tract exit through a single opening (the male urethra) in front, while the digestive tract exits through the anus. An anorectal malformation is a condition in which the rectum fails to separate properly during development and does not migrate into its proper position on the perineum. The anus and rectum lie in front of their normal location and are not located properly in the sphincter muscles of the pelvis. Anorectal malformations occur as a spectrum of abnormalities that ranges from very mild conditions in which the anus is situated only a few millimeters in front of its normal location, to severe anomalies with no visible anus. In the more severe abnormalities, there may be an abnormal emptying of rectum into the genitourinary tract, and associated malformations of the spine, genitalia, and kidneys.

Diagnosis of Anorectal Malformation

Anorectal malformations occur in both boys and girls. The diagnosis of an anorectal malformation can usually be made by inspection. When the baby is examined, the anus is absent or abnormally positioned. Ultrasound and x-ray tests may be used to help in the diagnosis. Additional tests will be obtained to look for associated abnormalities. Spine x-rays, kidney and spinal cord ultrasound, and occasionally echocardiography are all routine tests obtained in patients with anorectal malformations.

Treatment of Anorectal Malformation

The nature and severity of the anorectal malformation, as well as the presence of any associated conditions will determine the surgical treatment that is required. Some patients may be treated with a single operation as a newborn, while others will require operations in several stages. For patients with relatively minor abnormalities, a small procedure such as a cut back anoplasty may be all that is required prior to going home. For other patients, initial treatment consists of a colostomy and reconstruction is performed at a later date. A second operation is performed that separates the abnormal communication with the genito-urinary tract, and properly relocates the rectum within the sphincter muscles in the pelvis. Depending on the type of abnormality present, children may have this operation performed utilizing minimally invasive surgical techniques. Alternatively, a PSARP (posterior saggital anorectoplasty) procedure may be performed. Several months after definitive surgery, the colostomy is taken down, re-establishing gastrointestinal tract continuity

Follow Up of Anorectal Malformation

Following reconstruction, patients born with anorectal malformations must be followed frequently by their pediatric surgeons. For the more severe abnormalities, long-term continence issues are the major reasons for follow-up, while in the more mild conditions, patients and their families must work to insure that worsening constipation does not lead to poor continence. Many patients must be managed with bowel program designed to promote regular evacuations of the rectum to avoid ongoing soiling from chronic fecal retention. With attentive management, and rigorous compliance with prescribed regimens, most patients achieve social continence. Many patients will have normal continence. The Pediatric Surgeons of IAPS (Indian Association of Pediatric Surgeons) treat patients with anorectal malformations and provide long-term follow-up and management for patients and their families.

Arranging blood donor of rare blood groups( rh negative blood types

http://www.rarebloodgroups.org/

Arranging blood donor of rare blood groups( rh negative blood types ) is always a tough task as awareness on rare blood groups is still low and usually getting started on the search for rare blood groups draws a blank with most people.

Hundreds of blood units are required for patients suffering from blood diseases like anaemia, accidental blood loss, excessive destruction of red blood cells, thalassemia, haemophilia, leukemia and bone marrow transplant. There is an acute shortage of safe blood and statistics from most blood banks suggest rare blood groups (rh negative blood types) are very scarce.

The annual collection of blood in India is only 5.5- 6 million units against the requirement of about 8.5 million units.Delhi, Capital of India contributes only 24 per cent to the total voluntary blood donation and situation is more worse in other parts of the country

RareBloodGroups.Org is India’s 1st online registry website of rare blood type donors including AB Negative Blood Group, A Negative Blood Group, B Negative Blood Group, O Negative Blood Group and Bombay Blood Type.

* Auf NL200SH etablieren

* Ein halber Stapel High Society an winnings

* 15k VPP auf Stars erspielen

* Titanium-VIP auf Titan bleiben

MfG Basti

What a wonderful day

I am indeed. But today’s matter is another interest then me. It's the Bro-code. I must say I got inspired by Barney from How I Met Your Mother, so let me share the Bro-code with you:

The Bro Code:

Article 1/ The one rule: Bro's always before hoes.

Honor this rule cause it's the Holy Grail for every bro.

(2) If he's a real good and close bro to you listen to his opinion and follow it.

(3) Hands off from first grade family as well. (4) If you can’t avoid breaking the rule, you must either marry her or get your Bro laid with a really hot woman to make up for it.

Article4: You can critically judge the girls a bro hooks up with, but never ever interfere in his game. If she used to be a man, an animal or is related to him INTERFERE.

It sounds luxurious to have a one-hour wash of warm milk continuously poured over the body, but by this point in the treatment, it felt more like work than relaxation. My reserves were hitting bottom, it all felt all-consuming. What kept me energized enough to get myself to the clinic four times daily is in the knowledge that in the end, I’ll emerge rejuvenated. I hope.

The morning milk treatment was followed by an afternoon of two therapies. The first was the same treatment as in the second phase of pancha karma, where hot oil is drizzled into a makeshift well on the body; this time it was on my low back. This was followed by one of the most beneficial of the karma’s, the basti or medicated enema. It is especially indicated in Vata constitution or derangement, since the colon is the main site of Vata. Each day was a bit different in terms of the quantity and formula of the basti. Of all the treatments that were administered, the basti proved to be the most energizing. Afterwards, I felt a few sparks of life returning. What joy!

The final day, after the pancha karma was complete, I was prescribed an ayurvedic herbal concoction to be taken at bedtime. It’s action – to induce therapeutic diarrhea. This insures that the impurities that were loosened during the treatment are swiftly eliminated from the body. My experience did not, thankfully, match what the doctor described I could expect, namely 10-15 movements that may last most of the night. For me it was simple and fairly quick. Most everyone else I’ve spoken with has had the latter experience.

It’s been three days since the treatment ended. Each day I feel a little more energy. To be honest, I think some of this can be attributed to the liberation I feel with being on the other side of it now. I was mostly confined to my room and the clinic for nearly two weeks; now I feel free!

Siby explained that after pancha karma the body is open and in some ways this is when the healing begins. Therefore it is important to maintain a healthy diet (preferably the same one as during the treatment for the first few weeks) and to take care not to do anything to exertion, not to get overheated. This cancels out the few other destinations I considered before returning home next week, since the temperatures in both of those places are well above 100 degrees. Just the 12-hour bus travel from here to Delhi will prove taxing, unless I decide to splurge and fly back. It may be worth it; the trip from here is an arduous one.

I originally started this series of posts on my pancha karma treatment to do a review of Siby’s. I do have some concerns and criticisms which I’ll put in a separate, final post.

One of the world’s biggest computer and laptop manufacturers - Dell - has announced that as pa

One of the world's biggest computer and laptop manufacturers - Dell - has announced that as part of its corporate growth strategy - that it has begun installing Ubuntu 7.10 / Gutsy Gibbon, on its Dell consumer Linux PCs in the United States and will also make it available on the Inspiron 530 in England, France and Germany before the end of 2007.

Dell began offering consumer systems running Ubuntu in May 2007. Ubuntu’s Gutsy Gibson has been available on laptops since October.

The OEM and laptop testing teams of Dell have been testing the code and hardware drivers for several weeks and are ready to launch the Ubuntu 7.10 Laptop based on the free and open source Ubuntu Linux operating system.

“We believe Ubuntu 7.10 is a solid step forward <span style="font-weight:bold;">for both Linux enthusiasts as well as the mainstream consumer market</span>,” wrote Daniel Judd, Product Group Strategist on the Dell2Dell blog on Tuesday. “The answer is simple. With any operating system, we take gold code and take the time to do extensive testing on our systems to make sure that customers have as few issues as possible.”

Daniel Judd, Dell product manager has cited “cool” 3-D visual effects, easier desktop search for applications and the “ability to quickly switch between users and easily share a system with family members” as key new features of Ubuntu’s 7.10 release. He has also cited two minor Dell developed innovations much in demand by younger generation of laptop users — pre-installation of Flash and improved recovery options — as other significant enhancements for Gutsy Gibbon on Dell consumer PCs.

Judd also one key request was the ability to watch DVD movies from the Dell Linux desktop. “We totally agree and that’s why we now include built-in DVD movie playback with all Ubuntu 7.10 systems. The experience we wanted is simple — when you put a movie in, it plays. It is easy enough for a child and an example of the steps we are taking to make Ubuntu as enjoyable as possible.

Which Printer to Buy ? Which Printers are good with Ubuntu Linux Printing ?

Mera wala PRINTER kaun ? Latest, achha aur best printer mere Ubuntu ke liye kaun sa ?

Which printers are recommended for Ubuntu Kubuntu Linux users ?

For the Kanpur Lucknow based users of Ubuntu Linux computers one of the big questions to do some research on before buying a recommended printer from the Computer manufacturers or local assemblers - is whether the PRINTER being suggested works well with UBUNTU / KUBUNTU LINUX for English language printing ?
Please do ASK your computer supplier / manufacturer / local assembler if the printer they are suggesting for you to BUY, supports good black and colour printing with Ubuntu based Debian operating system ?
If the answer is NO - please do NOT buy that printer as it will be worth nothing to you once you buy it.
It is usually best to buy a LOW COST printer with good preloaded Linux Ubuntu DRIVERS.
If you want to check out if the printer which MAY LOOK GOOD but DOES NOT work well with UBUNTU then just tell the computer dealer to keep the printer in cold storage rather than expect you to buy a USELESS printer that you will not know where to DUMP and throw away.
Where to get the LATEST list of MODERN and best printers which work well with UBUNTU and KUBUNTU Linux ?
Check out at - http://www.linux-foundation.org/en/OpenPrinting

It is very important for Indian computer users to buy only printers which have good Linux printer drivers preloaded. There is NO POINT buying a printer if it does NOT work with Linux.
Do ask your computer dealer this question before buying a computer from them.
There are many reports of printers with a lot of marketing push behind them from computer dealers and manufacturers but that do not support Linux Ubuntu for Indian markets and Indian users.

Backwardness of Infrastructure or Backwardness of the Mind ?
For the Indian enthusiasts of Ubuntu and Linux it sometimes comes as a complete shock that a country which bursts into raptures when women are launched into outer space, a country that just freaks out when it sees fillums like Swadesh, Chak de India etc and a country which regards itself as one of the providers of choice of IT services, innovation and IT expertise to many parts of the world - is very silent when it comes to the question of IT infrastructure.
However if you have had ocassion to talk to some of the smartest Indian IT professors, working for well over decades in premier science and technology teaching and research institutes, one cannot help asking them a few questions on Linux and the Indian community :
1. For how many years have you been using Linux ?
2. For how many years have you been teaching Linux ?
3. How many world class conferences you have attended or hosted on Linux with lots and lots of government money ?
4. How many times you have given media interviews regarding IT innovation strategy for this country ?
5. How many years you have spent on accumulating experience on clusters of computing hardware ?

And then if you ask them one simple question as to why - if when a user in an Indian village wants to download Ubuntu Linux from an Indian server there is no Indian server offering free downloads - they just go suddenly quiet.
How long is this state expected to last ?
Just try going to the Ubuntu Linux download page and see (forget the world) - just check out which Asian countries have servers from which common people can download Ubuntu Linux from.
You will be amazed that all these high flying science and technology bureaucrats of India have not even found the time or the resources to offer a server for simple downloads.
And do you wonder what countries figure there ?
Well certainly not the so called big boss of IT - India. Nor the IT capitalists who profess to be socialists at heart.
There is no NASSCOM there. No IIT. No IIIT. No IIM
Then who is there ? -
Surprise of surprises - China, Uzbekistan, Thailand, Vietnam.
No Indian servers at all from any of the high flying prestigious IT computing hubs sprinkled all over India.
Is this issue symptomatic about ills and achievements of Indian Science and Technology, some fifty years after the dreams of public sector and science and technology revolution in India ?

It seems some kids and even some adult people ( not to talk of ignorant computer, laptop hardware dealers) have difficulties getting their own or customer broadband Internet connection from BSNL India - working with Ubuntu or Kubuntu Linux.

The BSNL Broadband connection costs only Rs 250/- per month and because it is an ADSL connection you can also use the telephone connection on the same phone line. You just need to phone up the BSNL Broadband support and ask them to give you the BSNL LAN details. Usually they are very helpful and will give you : Gateway Address, Primary DNS and Alternate DNS - All very simple - Just note down on piece of paper
The ADSL modem that they give like Sterlite which is a Chinese modem works very well with Ubuntu Linux in Kanpur Lucknow and all of Uttar Pradesh and no separate drivers are required. Just connect the Ethernet and phone lines to the modem. Connect the Ethernet cable to your AMD or Intel motherboard Ethernet port. A green and yellow light comes on if you see the back of your computer or laptop.

What next ? Once your Ubuntu or Kubuntu is installed from your ISO CD go to Applications - Accessories - Terminal and log in as root user. In Ubuntu root user is sudo

Type in the following in your shell - GNOME desktop shell also called GNOME Terminal :

sudo ppoeconf - Then enter the username that BSNL gave you. Next enter the password. Keep all the default settings regarding

sudo pon dsl-provider ( sudo poff dsl-provider - to disconnect Internet connection )

Nowadays most computer dealers will give you a Ubuntu or Kubuntu CD for just Rs 100 and you can install everything yourself. To install printer, keep the printer on while installing your Ubuntu.

Check with : plog ifconfig

Go to any website from your Firefox browser like : http://www.example.com

And hey, your Rs 250/- per month ADSL Broadband connection from BSNL is working at lightning speed in Kanpur Lucknow, Jhansi, Varanasi, Gorakhpur, Patna Moradabad. Nothing else to do. Just tune in to an Internet radio station from Ubuntu and enjoy the latest music and video clips.

Happy Ubuntu !! Everything is working fine !! Free legal Ubuntu !! I'm loving it baby !! And yes if somebody next door wants your help, please do help them out as wel. This is the spirit of Ubuntu in Kanpur and Lucknow. If your Ubuntu is working fine, please do spend some time to get your neighbours Ubuntu working also. No need to wait for IIT engineers. They will never come to you.
